Health and Consent Form

All attendees for Young Life trips must complete the Health and Consent Form. To complete the form online please contact your local staff person to receive a link to the digital consent form, providing the camp with important medical and insurance information in the event of an accident or illness. This form must be completed before departure for camp.

When completing the Health and Consent form, you may be asked for the following information:

Home Area

Young Life area number provided by your trip leader. You can also search by zip code.​​​

(Ex: VA37)

​Camp Name

Young Life Camp​​ that you or your child is attending.​

(Ex: Rockbridge)

​Camp Dates​

The camp dates that you or your child are attending.​

(Ex: June 16th - 21st)

​Health Insurance

Insurance company name, policy number, mailing address and phone.​

​Health History

Attendee ​health history including medication information and immunization records.

Health Form FAQs

The Health and Consent Form is not recognizing the participant's Date of Birth. What should I do?

Please verify that the Date of Birth entered matches the participant information used during registration and is entered in the correct format: MM/DD/YYYY​ (for example, 06/15/2012).

If the Date of Birth is entered correctly and you are still unable to proceed, contact your local Young Life area or the Young Life Connections Team at 719-381-1929 or helpdesk@sc.younglife.org​ for assistance.

Health Form ​ Questions

The participant's name on the Health and Consent Form is incorrect. What should I do?

Because this name is pulled from registration, it cannot be changed on the form itself.

Do not submit a form with an incorrect name. Doing so will invalidate the form, and you will have to start over. Please contact the Young Life Connections Team at 719-381-1929​ or helpdesk@sc.younglife.org​ to get the name corrected before you begin.

 

I'm having trouble uploading a file. What should I do?

The form only accepts PDF, JPG, and PNG files under 15 MB. If your file won't upload, try these quick fixes:

  • Check the file size and type: Ensure it is a PDF, JPG, or PNG, and under 15 MB.
  • Convert iPhone photos (HEIC): iPhones often save photos as HEIC files, which are not supported. Convert the photo to a JPEG or PDF before uploading.
  • Switch browsers or devices: Try using a different web browser (like Chrome or Safari) or switching from a phone to a computer.
  • Insert the image into a Word document and save it as a DOC file.
  • Need to upload multiple files? Click "Add Files" again to upload additional documents.

How do I make changes to the Health and Consent Form after it has already been submitted?

To make changes to a submitted Health and Consent Form, open the original email with the subject line "[Participant Name] - YL Health and Consent Form" and click the Health and Consent Form link.

After opening the form, verify the participant's last name and date of birth. You will then be able to update eligible information and resubmit the form.

My child is going to camp multiple times. Do I need to complete a form for each trip?

Yes, a separate Health and Consent Form is required for each camp trip, including Work Crew assignments.

When completing a new Health and Consent Form, some previously submitted information may carry over and be pre-filled for your convenience. Please review all information carefully and update it as needed before submitting the form for the current trip.

More than one family member attending camp.

If more than one child is traveling to a Young Life Camp from the same family, you can use the Copy Form function to copy basic information from one health form to another. To copy a health form for another family member:

1. Select​ the form you want to copy by selecting the blue bar. Make a Copy​ button.

2. Select​ Another Family Member. Make a Copy.

3. Verify​ your online signature.

  • ​​​ ​ ​ ​ A parent or legal guardian must sign the form if the camper is under the age of 18 or under the age of 19 for Malibu Club and Beyond Malibu. ​

4. Review and update ALL information​ for this family member.

  • ​​​ ​ ​ ​ Note - If you desire to upload ​ a copy of ​ an insurance card, be sure to do so BEFORE you submit the form.

5. Submit​ the form.​

6. Determine​ if a physician's signature form is required.​

How do I upload Immunization Records​?

An Immunization Record or Exemption Letter may be uploaded to an online health consent and release form while a parent is completing the form or it may be uploaded after the form is submitted.

Immunization records may be uploaded as PDF, JPEG or PNG files.

To upload​ an Immunization Record or Letter of Exemption after the form is submitted:

  • Log in​ to your Young Life health form account: healthform.younglife.org.
  • Select​ the health form by clicking the blue bar:​
  • Choose​ View Immunization Record:
  • Browse. Choose​ Immunization file on your computer. Save.​

Uploading a new Immunization Record will replace the current Immunization Record or Letter of Exemption that was previously uploaded or entered into the online health form.

What is a Registrant ID and Access Code?​

Don't have a Registrant ID or Access Code?

​​When a parent logs in to the online Young Life Health Consent and Release Form, they will be asked for a Registrant ID and Access Code. If​ the parent does not have these, please select the "Don't have a registrant id?" blue button. The parent will then be able to begin their camper's health form.
