2406-New York Wage Letter

These forms are mandatory for all staff working in New York. A copy of the form must be kept in Payroll records.​​

​Administrators are to fill in the current pay information on the appropriate form and have the staff person sign for approval at the time of new hire and any time a salary decrease is implemented. The sick time notification form is to be provided to staff, but a signed copy is not needed. Please send the wage document in along with the new hire paperwork/PAR.
