All forms must be completed by the volunteer leader applicant and returned to his or her area office. All completed forms are to be kept on file in the area office.
Every potential volunteer should complete two character references and one application.

Please note that the Faith & Conduct, Criminal Background Check and the Driver Questionnaire must be completed online by the volunteer leader. All leaders must be given access to the Staff Resources Site by setting them up through Volunteers Manager (staff access only).
