Description:
Amicus, Young Life’s International Exchange Student Ministry, has secured health and accident insurance for each student in the event that it is needed during the year. Coverage is provided by AIG. This coverage is for Amicus student exchange program participants only. The claims kit contains information needed for filing a claim.


Instructions:
All forms included should be completed by the student and the medical provider and mailed to the address provided on the form.
