6011 - Camp Physician Program Application & Insurability Form

Description:
Young Life's Camp Physicians Program operates in conjunction with the adult guest programs at our national camps. The physician and family are able to participate in a full week of camp, and the physician's services offer an additional degree of the professionalism in the medical services provided for campers.


Instructions:

To apply to serve as a physician at camp for one week, please fill out the application and insurability form and send the forms to the camp where you are interested in serving. Call the National Camping office with any questions: (719) 381-1844.
