6001 - Leader Resume and Recommendation

Description:
All persons interested in being a leader at camp must complete this form and give to their staff representative for his or her recommendation.


Instructions:

Applicant - Fill out your portion and give to your staff representative at least two weeks prior to the start of the camp. Include a photo of yourself.

 

Staff - Send to the appropriate camp office at least seven days prior to the start of the camp week.
