1734 - Waiver, Release and Indemnification Form

Description:

When signed by the child and the parent/guardian (if child is under 18), this form releases Young Life from all actions, suits, claims or demands related to injury or death from the specified activities in which the participant voluntarily participates. This form should be used when the child will be participating in activities that puts him/her at risk of injury or death.

Instructions:

Download and have all information and signatures prior to camp or the activity in which the child will be participating. Keep on file at your local area office. Call (719) 381 - 1959 with questions.
