1716 - Consent Release for Young Life Activity

Description:
In the event that a child or adult participant becomes injured or ill during a Young Life activity, having the parents or participants sign this form allows Young Life to take the injured person to the nearest medical facility or healthcare provider to administer the necessary treatment.

Instructions:

Form must be completed by parent or guardian prior to the start of a Young Life activity. Keep on record with your local area office. Contact Insurance and Risk Management with questions at (719) 381-1952.
