	1716 - Consent Release for Young Life Activity Allows Young Life to take a child or adult participant, who is ill or injured in a Young Life activity, to the nearest medical facility or healthcare provider to administer the necessary treatment.		1732 - Snow Skiing-Boarding Participant Enrollment Form Must be completed prior to a person attending a Young Life downhill snow skiing or boarding event.
	1733 - Snow Skiing-Boarding Enrollment Summary Must be filled out prior to any downhill skiing or boarding activity.		1734 - Waiver Release and Indemnification Form Releases Young Life from all actions, suits, claims or demands related to injury or death from activities in which the participant voluntarily participates. Available in Spanish.
	1735 - Permission to Drive If Under Age 19 Must be filled out if someone under the age of 19 is needed to drive for a WyldLife activity.		1738 - Consent Form for Medical Treatment in Canada Because Malibu Club is outside the United States, this form allows Canadian doctors to treat participants under Canadian guidelines.
	1739 - Map Release Grants authorization to Young Life to publish directions and/or a map to a personal residence on one of its websites.		1853 - Permission to Leave Camp Early via Plane This form must be completed and submitted to the camp office for campers leaving camp early via plane.
	1854 - Permission to Drive to Camp Must be signed by a parent or guardian if a kid will be driving to camp using a personal vehicle or riding with someone other than the Young Life group to camp.		1855 - Permission for a Camper to Leave Camp When it is necessary for a camper to be picked up from the camp property, use this form to get the parents' or guardians' permission.
	2012 Financial Statement and Ministry Statistics Young Life's financial statement and ministry statistics from fiscal year 2011-2012.		2223 - Electronic Funds Transfer Form and Information Brochure This form authorizes Young Life to charge a donor's bank account and credit card in the amount they wish to give to Young Life.
	2255 - Event Registration Form Use this registration form so people can sign up for a local Young Life event.		3215 - High School Application for Student Leadership Application for high school student leadership.
	6000 - Adult Guest Registration Adult guest or area staff person may complete this form for a stay at a camp.		6001 - Leader Resume and Recommendation All persons interested in being a leader at camp must complete this form and give to their staff representative for his or her recommendation.
	6002 - Adult Guest Recommendation This form must be completed by the area or regional director and accompany the Adult Guest Registration in order for an adult guest to be confirmed.		6003 - Application for Summer Staff Applicants who have at least completed their freshman year of college or are of equivalent age may submit this form to work as summer staff at a camp.
	6004 - Staff Recommendation for Summer Staff This recommendation must be completed by a Young Life staff member in order for someone to participate on summer staff.		6005 - Application for Work Crew Any high school student wishing to be on work crew during the summer must complete this application.
	6006 - Staff Recommendation for Work Crew This recommendation must be completed by a Young Life staff member in order for a student to participate on work crew.		6007 - Camping Health, Consent and Release Forms Used to gather pertinent information in the case of accident or illness at camp. Available in English and Spanish.
	6009 - Guest Consent Release Form for Outside Groups Using a Young Life Camp In the event of an accident or illness, Young Life needs the information on this form from all guests at a Young Life camp.		6011 - Camp Physician Program Application Use to apply to serve as a physician at camp for one week.
	6012 - Adult Guest Host Resume Allows prospective adult guest hosts to indicate their interest and to submit necessary information.		6022 - W Form Registration for Winter Camp For all school season and weekend camps, record the participants' information on this form.
	6055 - Properties Yearlong Intern Application An application accepted by all Young Life camps that currently have a yearlong internship program.		6201 - Housing Request Form Allows leaders to establish housing assignments for camp.
	6202 - A Form Registration for Summer Camp Complete list of participants, fees and scholarships required for each camp trip.		A Living Legacy This video introduces Young Life and details the 5 C's of Young Life.
